Decoding Constantly erroring...

How do i fix this. i LOVE this program but keep running into various decoding issues, i'm assuming if i can get an answer to this problem it will solve alot of my other decoding errors, in which i just deleted files before checking out the log... 

Any help would be greatly appreciated. Thanks in advance.
